When you become a patient connected to Humber River Family Health Team, you gain access to much more than your family physician.
Our multidisciplinary team provides OHIP-covered services at no additional cost when clinically appropriate. Eligible patients may be able to access care across our Family Health Team locations based on the service they need and appointment availability.

Our Dietitians work with individuals and groups to improve eating habits and address the nutritional needs of people with complex health problems (such as dyslipidemia, hypertension, diabetes, heart disease and obesity) to improve health outcomes. Our Dietitians are Certified Diabetes Educators who offer both individual consultations and group classes focused on understanding and managing diabetes to promote healthy living and prevent diabetes-related complications.

As part of the interdisciplinary team, they support individuals affected by mental health via psychotherapy (i.e. cognitive behavioural therapy), by providing resources, and connecting to community programs. Types of supports may include the following:
Our Chiropodists specialize in the recognition, assessment, treatment and ongoing care of the foot. They are extensively trained to help patients eliminate foot pain and maintain healthy feet.

The administration team works to ensure care is delivered in an efficient and patient centred way. They include staff in various areas who do a variety of roles, including answering phones, and booking appointment, organizing and evaluating programming, quality improvement planning and reporting, and overall management.

A Family Health Team provides a team-based approach to primary care. Rather than relying primarily on episodic walk-in clinic visits, you can build an ongoing relationship with a family physician and a broader healthcare team that can support you over time.
Your physician can connect you with other members of the team when needed, helping you access specialized support for medication management, nutrition, mental health, chronic disease, foot care and navigating the healthcare system. Services may also be available across Humber River Family Health Team locations based on the service you need and appointment availability.
