TORONTO | October 28, 2026 — Humber River Family Health Team (HRFHT) is expanding access to primary care across Northwest Toronto following a $2.3 million provincial investment through Ontario’s Interprofessional Primary Care Team (IPCT) initiative. The funding will support attachment of up to 5,600 additional residents to ongoing primary care and approximately 17.8 additional full time equivalent clinical and administrative positions.
With approximately 45 affiliated family physicians and a patient population of approximately 50,000, HRFHT is among the largest team-based primary care providers serving Northwest Toronto. Its expansion brings access to more care into high-risk communities while strengthening services at already established clinical sites.
One Health Team. Multiple Locations. No Closed Doors.
HRFHT’s “No Closed Door” approach means that patients attached to the organization are connected to a network, not limited to one office. Depending on clinical need, eligibility and appointment availability, patients can access care at different HRFHT locations, including same day appointments, evening and weekend clinics, and referrals to interprofessional healthcare providers such as nurses, nurse practitioners, social workers, dietitians, pharmacists and chiropodists.
When patients join Humber River Family Health Team, they are not simply joining a doctor’s office. They are connected to an entire network of healthcare professionals, locations and services across Northwest Toronto. Our goal is to ensure the right care is available at the right time and at the right location.

Care That Extends Beyond Traditional Clinic Walls
HRFHT’s established clinical footprint includes locations along Dixon Road, Weston Road, Wilson Avenue and the Keele and Finch corridor. The expansion also supports new and developing community-based services with partners including the I Help Centre, Progress Place and Across Boundaries.
By working with trusted neighborhood organizations, HRFHT aims to bring primary care closer to residents who may face barriers related to transportation, cost, mobility, mental health, or navigating the healthcare system. Community based sites create additional places where individuals can connect with care and be supported in accessing other services when needed.
A Wider Range of Professionals Working Together
A family physician or nurse practitioner is a vital part of primary care, but many health needs benefit from a broader team. HRFHT brings together physicians, nurse practitioners, registered nurses, registered practical nurses, social workers, dietitians, pharmacists, chiropodists and administrative and navigation professionals.
These healthcare professionals work together to support preventative care, chronic disease management, mental health and wellbeing, medication reviews, nutrition counselling, foot care and health education. Connecting patients across sites and disciplines helps support continuity of care while giving them more options for timely assistance.
Welcoming New Patients
HRFHT is accepting expressions of interest from residents seeking a family doctor and access to team based primary care. Its established clinical locations include 245 Dixon Road, 2050 Weston Road, 1017 Wilson Avenue, and 1275 Finch Avenue West. The availability of physician attachment and particular services varies by location.
Residents can visit www.hrfht.com/accepting-new-patients/ to learn about the registration process and available services. Once attached, eligible patients can benefit from the wider HRFHT network, including interprofessional services and after-hours clinical services, subject to appointment availability and clinical appropriateness.
Building The Next Chapter of Primary Care
HRFHT’s expansion reflects a commitment to a more connected and accessible system of primary care in Northwest Toronto. By investing in healthcare professionals, partnerships and additional service locations, the organization is working to ensure that more people have an ongoing relationship with primary care and clear pathways to the support they need.
The message is simple: one health team, multiple locations, and a commitment to helping patients find an open door to care.
