Patient Feedback and Complaints

The Humber River Family Health Team is committed to ensuring that every patient
feedback and complaint is received and addressed respectfully, promptly, and
appropriately, regardless of the level of concern.
Our organization includes two integrated groups: the Humber River Family Health
Organization (FHO), which consists of 43 physicians and their administrative staff
across six sites, and the Humber River Family Health Team (FHT), which includes
management, interprofessional health providers, and administrative staff across three of
those six sites. Together, the FHO and FHT work collaboratively to provide
comprehensive patient care.

While we strive to always provide the highest standard of care and service, concerns
may occasionally arise. We want patients to know that feedback and complaints
regarding any member of staff may be submitted and will be reviewed with appropriate
attention and priority.
Submitting feedback or a complaint will not negatively affect a patient’s access to care,
quality of care, or relationship with their care team.

you are experiencing a medical emergency, call 911 immediately. For urgent care
needs, please contact your clinic directly.
Information submitted through this form will be reviewed respectfully and handled with
care. Details will be shared only with the appropriate person or persons involved in
reviewing and responding to the feedback or complaint.

FHO/FHT Patient Feedback/Complaint Escalation Process.

This section outlines the escalation process for patient complaints involving FHO and FHT staff to support timely review, appropriate accountability, and consistent handling across sites. 

All complaints will first be directed to the Humber River Family Health Team Executive Director, Nathan Stern, and will then be assigned for review as outlined below.
