Topics: Infant vaccine schedule (2, 4, 6 months), Caregiver vaccines (Tdap), Mental health & parenting support, Local health resources.

Target Audience: Expectant parents & caregivers; New parents; Families in local catchment.

Key Benefits: Builds trust with care team; Preventive care; Connects families to resources; Space for Q&A.

Schedule / Format: 2 sessions, 45 minutes each; Twice monthly; Hybrid delivery; Complements Black Creek’s program.

Facilitators: Registered Nurses, Nurse Practitioners, Social Workers.
