Part 2: COPD/Asthma- Dixon

Overview: Rotating sessions on self-management for
chronic diseases: Diabetes, COPD & Asthma,
Hypertension & cardiovascular health. Includes blood
pressure control, medication adherence, heart-healthy
living, and respiratory care. COPD/Asthma patients may
also access services from a Pharmacist (spirometry)
and a Nurse Practitioner (vaccinations).

Target Audience: Patients with chronic conditions;
Adults managing multiple conditions; Caregivers.

Key Benefits: Inclusive for multiple conditions;
Encourages open discussion; Builds patient confidence;
Links to follow-up care.

Schedule / Format: 60 minutes per class; 3 rotating
topics (weekly); Every other month; Hybrid (in-person &
Zoom).

Facilitators: Dietitians, Nurse Practitioners,
Pharmacists.
