Part 2: COPD/Asthma – Weston

Target Audience: Patients with chronic conditions, adults managing multiple conditions, and caregivers.

Key Benefits: Inclusive for multiple conditions; Encourages open discussion; Builds patient confidence;
Links to follow-up care.

Facilitators: Dietitians, Nurse Practitioners, and Pharmacists.

Schedule/Format Highlights (Rotating): 60 minutes per class; 3 rotating topics (weekly); Every other month; Hybrid (in-person & Zoom).

  • Tuesdays (Weston): 10:00 AM – 11:00 AM.
  • Wednesdays (Dixon): 5:30 PM – 6:30 PM.

With our patients’ health in mind, we offer FREE programs, services and classes.  The aim is to promote, prevent and educate our patients (and community members) in a safe and caring environment, lead by our Health Care Professionals.  Many of these programs are designed to move you forward in your health care journey.

You do not need to be a patient to attend any of these free classes. Everyone is welcome to attend
