Who is On Our Family Health Care Team?

A Nurse Practitioner (NP) is a nurse with advanced university education who works both independently and in collaboration with other health professionals to provide you with quality health care services.

NP’s can:

  • Diagnose and treat acute and chronic illness 
  • Prescribe medications however, controlled substances cannot be prescribed and scope is limited.
  • Order and interpret diagnostic tests
  • Provide education and counselling

  • Our Registered Nurses (RN) and Registered Practical Nurses (RPN) work with physicians and other interdisciplinary health team members to promote health and prevent disease.

RN’s and RPN’s can assist with and perform the following roles:

  • Routine immunizations/injections
  • TB Screening
  • STI (Sexual Transmitted Infection) and contraception education 
  • Cervical cancer screening and education
  • Staple/suture removal and dressing changes
  • Ear syringing
  • Group health education programs

Our Dietitians work with individuals and groups to improve eating habits and address the nutritional needs of people with complex health problems (such as dyslipidemia, hypertension, diabetes, heart disease and obesity) to improve health outcomes. Our Dietitians are Certified Diabetes Educators who offer both individual consultations and group classes focused on understanding and managing diabetes to promote healthy living and prevent diabetes-related complications.

Services offered include: 

  • Nutrition and lifestyle education.
  • Injection technique for insulin as well as other injectable diabetes medications. 
  • Insulin start and adjustment.
  • Glucometer teaching.
  • Application of glucose sensors and interpretation of the CGM reports.
  • Navigating compassionate programs for accessing diabetes medications and supplies.

Our Social Workers are regulated and trained healthcare professionals. 

As part of the interdisciplinary team, they support individuals affected by mental health via psychotherapy (i.e. cognitive behavioural therapy), by providing resources, and connecting to community programs. Types of supports may include the following:

  • Patient specific psychotherapy, counseling, and education
  • Life transitions – work, school, retirement
  • Relationships
  • Perinatal mental health
  • Immigration/refugee
  • Financial stress
  • Stress management
  • Regulating Emotions
  • Group education/support (e.g. stress management, self-care)
  • Pinpointing stressors and sources of unhappiness
  • Help develop individualized coping plans to improve mental health and well being
  • Supportive paperwork such as government assistance program forms
  • Referrals and connection to external community resources

Our Chiropodists specialize in the recognition, assessment, treatment and ongoing care of the foot. They are extensively trained to help patients eliminate foot pain and maintain healthy feet.

A Chiropodist may perform the following:

  • Diabetic foot assessments and wound care
  • Treating corns, calluses, nail abnormalities, etc.
  • Ingrown toenail surgeries

Pharmacists are regulated health care professionals who offer a range of health care services and are medication experts. They work both independently and with the health care team to address and help manage chronic diseases.
 
The Ontario government has recently  expanded the scope of practice that allows pharmacists to assess and prescribe medication for 19 minor ailments.
 
Our clinical pharmacist may perform the following:
  • Medication reviews
  • Reconciling medication changes (e.g. after being discharged from hospital)
  • Chronic disease management
  • Education and counselling on medications and diabetes management
  • Deprescribing chronic medications
  • System navigation and financial aid for medications
  • Smoking Cessation
The pharmacist can prescribe medication for certain minor ailments, including Urinary Tract Infections, Cold Sores, Conjunctivitis (pink eye), and Allergic Rhinitis (nasal allergies).

The administration team works to ensure care is delivered in an efficient and patient centred way. They include staff in various areas who do a variety of roles,  including answering phones, and booking appointment, organizing and evaluating programming, quality improvement planning and reporting, and overall management.
