HRFHT offers a wide range of free health and wellness programs for the community.

All programs are open to everyone and you do not need to be a patient of a doctor in our team to participate.

Chronic Condition Education Series

Self-Management & Heart Health

Join our rotating sessions on self-management for chronic diseases, including Diabetes, COPD, Asthma, and Hypertension. We cover essential topics like blood pressure control, medication adherence, and heart-healthy living.

Frequency: Weekly rotating topics; runs every other month (60 mins).

Prenatal & Early Family Health Education

Infant Health & Parenting Support

This program supports infant health and early parenting with a localized care model. Topics include infant and caregiver vaccine schedules, mental health support, and navigating local resources.

Frequency: Two 45-minute sessions; held twice monthly.

Weight Management & Healthy Eating

Nutrition & Culturally Tailored Wellness

Attend monthly nutrition sessions tailored to specific communities. We explore weight management tools, youth body image, and culturally appropriate meal planning to build healthy habits.

Frequency: Monthly sessions (60 mins); rotates by location.

Support for Seniors

Falls Prevention & Mental Wellness

A progressive series combining nutrition, fall prevention, and depression support. Learn about safe movement, balance, footcare education, and strategies to reduce loneliness while aging in place.

Frequency: Quarterly 4-week series (60 mins per session).

Targeted Mental Health Programs

Life Stage Mental Health Education

Structured education covering mental health across life stages. Topics range from early intervention for youth mood disorders to anxiety management for adults and sleep therapy.

Frequency: Twice yearly; 3-session series (60 mins per session).

Support for Newcomers

Settlement & System Navigation

Workshops designed to address settlement stress and well-being. We cover cultural adaptation, emotional resilience, overcoming language barriers, and navigating health services.

Frequency: Twice yearly; 3-session series (60 mins per session).

Lung Health & Smoking Cessation (OMSC)

Quit Smoking or Vaping Support

Support for patients aged 12+ to quit smoking or vaping using the Ottawa Model. Includes personalized counseling, nicotine replacement therapy (NRT), and progress monitoring.

Frequency: Year-round; ongoing individualized support.

Caregivers Support Group

Emotional Support & Resources

A dedicated group for family and unpaid caregivers. We offer emotional support and share resources on rotating topics such as cancer, developmental disorders, mental health, and dementia.

Frequency: Monthly sessions (60 mins) with rotating topics.

Weight Loss Support Group

Lifestyle Change & Accountability

Monthly sessions supporting individuals exploring weight loss and medication considerations. We discuss the causes of weight change and diet approaches while providing peer accountability.

Frequency: Monthly sessions (60 mins).

Women’s Health Education Series

Health Across Life Stages

Education tailored to women and those assigned female at birth. Topics include HPV awareness, preconception and fertility, prenatal health, and menopause management.

Frequency: Quarterly; 4 sessions per cycle (60 mins each).

Sexual Health Education Series

Inclusive Prevention & Safety

A comprehensive and stigma-free series on sexual health. We cover STI prevention and testing, birth control options, safe sex practices, and healthy relationships.

Frequency: Quarterly; 3 sessions per cycle (60 mins each).

Addiction & Mental Health Education Series

Recovery & Risk Factors

Foundational education on the link between mental health and addiction. Learn about diagnoses, substance use, treatment options, and support for caregivers and families.

Frequency: Quarterly; 3-session series (60 mins each).
