About Us

What is a Family Health Team?
A Family Health Team brings together doctors, nurses, pharmacists, social workers and other health care professionals who work collaboratively, using their combined expertise to provide care focused on early intervention and long term health.
Services are delivered in partnership with Doctors, Nurse Practitioners, Registered Nurses, Dieticians, Social Workers, Chiropodist, Pharmacist and Administrative Support Staff.
You are part of a Family Health Team
Continuous Care
Comprehensive, holistic care in-house that is OHIP-funded. Your family doctor continues seeing you in regular office hours.
After Hours Care
You can access an After Hours Clinic for urgent concerns during evenings and weekends.
Health as a Team
If you see another provider within the team, your family doctor is kept informed to ensure continuity of care.
Our Board of Directors
The Board of Directors includes both physicians and community members who work together to provide high-quality care and strong governance in support of patients and their families.
We are currently seeking two individuals to join the Board of Directors.
The Board provides strategic oversight and ensures HRFHT continues to deliver accessible, patient-centered services to the community.

Our team includes family physicians, nurse practitioners, registered nurses, dietitians, social workers, pharmacists, chiropodists, and administrative support staff. We work together to ensure patients receive the right care, at the right time, in the right setting.

In addition to clinical care, HRFHT offers a wide range of free health and wellness programs for the community.
All programs are open to everyone and you do not need to be a patient of a doctor in our team to participate.
These programs are led by experienced health professionals who provide reliable information, guidance, and support in a welcoming environment.
